    It's happened at least twice and maybe three times over the last few yeas. This is the first fatality. I read an article about it the other day and the source said something along the lines of "this is just a random/act of god incident. Then I remembered it's happened before. Massachusetts roads are horrendous in general, this takes it to the next level. But is this a big dig debacle or mass roads in general problem?
    I remember back around 2001 I was working at a huge liquor store and a guy with a bectal (or whoever was doing the big dig) hat on and I threw my wallet on the counter and said "Do you want this now, or would you like it later?"
    My grandchildren will still be paying for that disaster.

    Great question. They welded down the cover that almost killed someone a while back. Why didn't they weld down this one as well?

    And even if it weren't welded down, what is the force causing it to fly three or four feet straight up in the air?

    I once saw a manhole cover fly up at least 20 feet in the air in NYC late one night. Scared the shit out of me!

    I don't think they're going straight up in the air. I read they were "kicked back". Seems like half the man hold covers and drainage grates in the Commonwealth have sunked down several inches. So instead of being even with the surface, they're like super potholes. I wonder if this was one of those. Maybe the force of a large truck hitting it made it bounce and then got spit out by the tire?
