It's a whole ordeal - she'd have to call a lawyer...Actually I guess that's it, the lawyer could handle everything.

I thought she would have jumped at the chance to change her name to Stern. She knows that's her only "claim to fame". Why else does she mention that she's married to Howard Stern at every given moment?
I just listened to what I thought was the whole interview on 101.....either I'm blacking out, and I don't drink by the way, or that was not in the interview!

The fact that she hasn't changed it speaks volumes about what kind of person she is. First off, I think we can all agree that she's neither a feminist nor some type of deep-thinking, principled woman. The time she and Howard shared prior to getting married she loudly proclaimed (on 60 Minutes) what a subservient woman she was--how everything was all about Howie and how she was perfectly happy with that.
So she was playing the role of a Stepford wife, but when her big moment arrived, she chooses to keep her own (difficult to pronounce) last name? That decision isn't congruent with the way she presented herself previously. And to claim it is a hassle to change it now--well that's clearly a lie. As others have noted, she and Howard are super-wealthy and have a team of attorneys at their disposal. A simple phone call and all she'd have to do is sign some documents the next day and it would all be handled for her--painlessly.
So we've established that she misrepresented herself prior to the marriage and she now lies about why she still hasn't followed through. It's not that she should have taken his name--it's the fact her refusal to do so is incongruent with how she sold herself to Howard. If I were him, I'd have some serious reservations about believing anything she said.
It's interesting that Howard brought up this issue on the air. He probably doesn't have the guts to confront her about it in private.
He sounded miserable when he was talking about how much he loved Beth and implied that he didn't want to "make waves" with her. It was pathetic.
