Not sure, but Freudian therapy obviously works if you want to transform a quasi-hetero male into a screeching, flaming, flitting, scarf-wearing closeted homosexual. It takes 20 years to see the full effects of therapy, but it works.

I think the therapist just counts the money he's getting and HAS gotten. Millions over the years. What has come out of it? A divorce and a marriage to a gold digger.

It's probably an endless list of complaints, followed by the therapist saying "they're just jealous of you, Howard." Whatever they do, it isn't working because Howard is incapable of enjoying himself and his relationship with Beth is comical.

To answer the OP's question in a somewhat serious manna, I actually think that Wig expressed his frustrations to his therapist at about the 18 year mark, and demanded he get better soon or he would quit therapy. Assessing the needs of his client (i.e., not wanting to lose his cash cow), the therapist decided he would finally give up on trying to treat severe narcissistic personality disorder with Freudian methods (which is considered almost untreatable under the best circumstances) and concentrate on helping Howard "fake it"--in other words, give him the cognitive-based therapy he probably would have benefited from all along.
This is around the time that Wig was talking on-air about the guy with Aspergers who learned how to interact with people by emulating Howard--an irony that must not have been lost on him.
Since the approach of his therapy has changed, Wig has come to be "okay" with his narcissism disorder, embracing it with the help of his enabling wife (herself a myriad of complex psychological problems). We now see a Howard that does not fight his NPD, but embraces it--and pretends, like the alien insect humanoid that he is, that he's like the "rest of us."
Either that, or "he's a fucking faggot."
